A known complication following palliative surgical repair for hypoplastic left heart syndrome is right ventricular dysfunction.  Bone marrow-derived mesenchymal stem cells (MSCs) may be the treatment of the future for these patients.  MSCs are an “undifferentiated population of cells capable of multipotent differentiation.” This study evaluated whether human MSCs could preserve right ventricular (RV) function and promote RV remodeling in a porcine model.  Ten neonatal swine underwent pulmonary artery banding followed by injection of MSCs (n=5) or placebo (n=5) into the RV free wall.  The swine treated with MSCs demonstrated the following significant findings compared to placebo controls 4 weeks post-banding.

  1. Decreased RV dilation with increased RV fractional area change
  2. Decreased tricuspid regurgitation (measured by width of tricuspid vena contracta)
  3. Improved myocardial strain
  4. Increased neovessel formation
  5. Increased proliferation of cardiomyocytes and endothelial cells
  6. Decreased RV hypertrophy
  7. Decreased RV myocardial fibrosis

Although 4 weeks post-banding there were very few MSCs remaining in the RV myocardium, injection of MSCs appears to have had a beneficial effect in the porcine model with RV pressure overload.  Mesenchymal stem cell treatment may become the treatment of the future in patients with right ventricular dysfunction.

  1. Tricuspid annular plane systolic excursion (TAPSE) in adult pulmonary hypertension studies correlates with cardiac output and predicts mortality.
  2. This pediatric study found that TAPSE was reduced in patients with pulmonary hypertension and history of repaired congenital heart disease. In this patient population the TAPSE did not correlate with functional status and is most likely secondary to post-operative changes and not a reflection of poor right ventricular function due to pulmonary hypertension.
  3. Most patients with pulmonary hypertension without history of repaired congenital heart disease in this study had a normal TAPSE. This finding is most likely due to later right ventricular failure in children compared to adults with similar pulmonary artery pressures.

 

Commentary from Dr. Ginnie Abarbanell (Atlanta), section editor of Pediatric Cardiology Journal Watch: Tricuspid annular plane systolic excursion (TAPSE) in adult pulmonary hypertension studies correlates with cardiac output and predicts mortality.  There are limited studies in pediatrics that evaluate TAPSE and pulmonary hypertension (PH).  This study from Colorado evaluated TAPSE in 84 children and young adults with PH and 315 healthy children.  This was an association between TAPSE and echocardiographic measures including septal flattening, subjective right ventricular (RV) function, RV fractional area change and RV S.  TAPSE did not correlate with the estimated RV pressure by echocardiography.  Interestingly, this study found that TAPSE was significantly lower in patients with pulmonary hypertension and history of repaired congenital heart disease (CHD) involving bypass compared to normal children and children with other causes of PH.   This reduced TAPSE in patients with pulmonary hypertension and history of CHD did not correlate with functional status and the authors conclude that the decreased TAPSE is most likely secondary to post-operative changes.  Comparatively, there was a trend towards lower TAPSE with longer duration of disease but many patients with PH had normal TAPSE z-scores.  This finding is most likely due to later RV failure in children than adults with similar pulmonary artery pressures.

Commentary from Dr. Ginnie Abarbanell (Atlanta), section editor of Pediatric Cardiology Journal Watch: There are published studies that report a high risk of death among infants of very low birth weight born with serious congenital heart disease (CHD).  This retrospective cohort study examined the mortality of infants born premature at <34 weeks gestation with critical or moderately severe CHD between 2007 and 2012 at the University of Michigan.  Critical CHD was defined as requiring surgery as a newborn and moderately severe CHD requiring surgery by 6 months of age.  There were 54 infants that met criteria.  25 of the 54 (46%) survived to hospital discharge.  Among the non-survivors, 74% died prior to surgery at a median of 24 days.  Those infants that underwent surgery had a 75% survival.  On multivariate logistic regression, gestation age and presence of an extracardiac abnormality were independent risk factors for mortality.  A premature infant with CHD born with no other abnormalities was 11.7 times more likely to survive compared to a premature infant with CHD and an extracardiac abnormality.  Additionally, the odds of survival increased by a factor of 1.7 for every week increase in gestational age.  In this study the primary indications for delivery, premature prolonged rupture of membranes (PPROM), chorioamnionitis or hydrops fetalis were not significant predictors of neonatal mortality.  However, the authors also suggest given the small sample size there may not have been sufficient power to detect a difference.

This is a prospective, randomized trial of postoperative tight glycemic control vs standard care in 980 children undergoing cardiac surgeries at 2 centers. The objective of this study was to evaluate the association of postoperative tight glycemic control and hypoglycemia in children undergoing cardiac surgery with neurodevelopmental outcomes at 1 year of age. Neurodevelopmental outcomes were assessed at 9 to 18 months using the Bayley Scales of Infant and Toddler Development, Third Edition (Bayley-III), the Adaptive Behavior Assessment System, Second Edition, the Ages and Stages Questionnaire, Third Edition, and the Brief Infant Toddler Social-Emotional Assessment. A total of 237 patients had a neurodevelopmental follow-up at a mean age of 13 months. No significant differences were found in the Bayley-III and Adaptive Behavior Assessment System, Second Edition composite scores or percentage at risk based on the Ages and Stages Questionnaire, Third Edition and the Brief Infant Toddler Social-Emotional Assessment between the tight glycemic control vs standard care cohort. Eight Patients who experienced moderate to severe hypoglycemia had lower Bayley-III composite scores compared with patients with no to mild hypoglycemia, even after controlling for factors known to be associated with poorer neurodevelopmental outcomes.

Commentary from Dr. Shaji Menon (Salt Lake City), section editor of Pediatric Cardiology Journal Watch: This is a retrospective study (January 2005 to June 2014) of 113 patients (55% male; mean age, 17.9 years; age range, 5–21 years) who underwent ultrasound contrast (UC) echocardiography for left ventricular opacification or stress wall motion and perfusion analysis. The study included patients in the following diagnosis categories: Congenital heart disease (30%), acquired heart disease (21%), and other (suspected cardiac complications of disease of other organ systems; 49%). In this 12 patients (11%) with right ventricular systolic pressures > 40 mm Hg received UC without complications. In four of these patients the highest right ventricula/right atrial gradient (tricuspid regurgitation Doppler signal) was better visualized with UC. Myocardial perfusion abnormalities were seen in 13 patients (12%); four had confirmed correlations by angiography or magnetic resonance imaging. There were 13 (12%) instances of adverse events or reported symptoms during UC echocardiography. All symptoms and events were transient, all patients completed protocols, and there were no immediate sequelae.
