January 2014 CHD Surgery Articles

1. Effects of preoperative hypoxia on white matter injury associated with cardiopulmonary bypass in a rodent hypoxic and brain slice model.

Agematsu K, Korotcova L, Scafidi J, Gallo V, Jonas RA, Ishibashi N.

Pediatr Res. 2014 Jan 31. doi: 10.1038/pr.2014.9.

Comment: This study further strengthens the deleterious effects of cardiopulmonary bypass in CHD and identifies some potential therapies of promoting white matter regeneration by stimulating oligodendrocyte progenitors after earlier surgery using deep hypothermia. Neurocognitive delays in ACHD patients is quite prevalent and studies like these may help change the early and long term neurologic outcomes.

2. Alveolar recruitment manoeuvre is safe in children prone to pulmonary hypertensive crises following open heart surgery: a pilot study.

Amorim ED, Guimaraes VA, Carmona F, Carlotti AP, Manso PH, Ferreira CA, Klamt JG, Vicente WV.

Interact Cardiovasc Thorac Surg. 2014 Jan 30.

Comment: Simple maneuver in the OR just after skin closure to recruit more alveoli and improve Pa O2/saturations with slight transient elevation of mean pulmonary arterial pressure. We can drop this article – the Study does not have a clear and strong message. It does not conclusively mention if this maneuver reduces the incidence of atelectasis, post op stay, pulmonary crisis etc.

3. Is there a role for mechanical valve prostheses in pulmonary valve replacement late after tetralogy of Fallot repair?

Abbas JR, Hoschtitzky JA.

Interact Cardiovasc Thorac Surg. 2014 Jan 30.

Comment: This review may be of less value in the era of percutaneous pulmonary valves however it gives an opportunity to think on this option as a last resort. The authors have very well complied pulmonary mechanical valve series published so far.

4. Marked changes in platelet count and function following pediatric congenital heart surgery.

Bønding Andreasen J, Hvas AM, Ravn HB.

Paediatr Anaesth. 2014 Jan 28. doi: 10.1111/pan.12347.

Comment: Both platelet count and platelet function were significantly reduced after CPB in children undertaken correctional heart surgery. Duration of CPB and hypothermia was associated with significant changes in platelet function.

5. Early Complete Repair of Pulmonary Atresia With Ventricular Septal Defect and Major Aortopulmonary Collaterals.

Watanabe N, Mainwaring RD, Reddy VM, Palmon M, Hanley FL.

Ann Thorac Surg. 2014 Jan 27. pii: S0003-4975(13)02847-6. doi: 10.1016/j.athoracsur.2013.10.115.

Comment: The data demonstrate that early complete repair of PA/VSD/MAPCAs can be accomplished with low mortality and excellent postoperative hemodynamics. These early hemodynamic results are maintained at medium-term follow-up. We conclude that early complete repair is an appropriate choice for this highly select subgroup of patients.

6. Risk stratification and outcome of cardiac surgery for patients with body weight <2,500g in an asian center.

Chen JW, Chen YS, Chang CI, Chiu IS, Chou NK, Huang HH, Huang CH, Huang SC.

Circ J. 2014 Jan 24;78(2):393-8. Epub 2013 Nov 21.

Comment: Cardiac surgery performed on infants with low body weight is generally a complex procedure, but the results are acceptable. The risk was higher than that for patients with higher body weight in RACHS-1 category 3 and 4. Further investigation to improve the outcome of this high-risk group is needed.

7. [Heart and combined heart-lung transplantation : Indications, chances and risks.]

4. Pregnancy-related obstetric and cardiologic problems in women after atrial switch operation for transposition of the great arteries.

Trigas V, Nagdyman N, Pildner von Steinburg S, Oechslin E, Vogt M, Berger F, Schneider KT, Ewert P, Hess J, Kaemmerer H.

Circ J. 2014 Jan 24;78(2):443-9. Epub 2013 Dec 10.

Comment: Pregnancy is usually well-tolerated, but outcome is unforeseeable and life-threatening problems can occur. These women belong in cardiac care conducted by experienced congenital cardiologists, who systematically check for typical residua. The pregnancy should be planned and gynecologists/obstetricians with special expertise integrated into the consultations. During delivery a congenital cardiologist, and an anesthetist experienced in congenital cardiology, should be present for possible severe cardiac events.

1. Reference intervals for the echocardiographic measurements of the right heart in children and adolescents: a systematic review.

Lemmer Hunsinger CE, Engel ME, Stanfliet JC, Mayosi BM.

Cardiovasc Ultrasound. 2014 Jan 29;12(1):3. doi: 10.1186/1476-7120-12-3.

Comment: Reliable reference data are lacking for important echocardiographic measurements of the RV in children and adolescents, especially for sub-Saharan Africans. Negative study however with a clear message.

2. The potential and limitations of plasma BNP measurement in the diagnosis, prognosis, and management of children with heart failure due to congenital cardiac disease: an update.

Cantinotti M, Law Y, Vittorini S, Crocetti M, Marco M, Murzi B, Clerico A.

Heart Fail Rev. 2014 Jan 29.

Comment: This review article highlights the diverse range of natriuretic peptides in children with CHD and its role in certain population.

Comment: Good review on CHD and CHF with recent advances.

6. Bicarotid Trunk: How Much Is “Not Uncommon”?

Reinshagen L, Vodiskar J, Mühler E, Hövels-Gürich HH, Vazquez-Jimenez JF.

Ann Thorac Surg. 2014 Jan 27. pii: S0003-4975(13)02849-X. doi: 10.1016/j.athoracsur.2013.12.014.

Comment: The prevalence of bicarotid trunk of 17.6% among patients with aortic coarctation and 15.2% among the entire population studied made it not at all a rare phenomenon. Because bicarotid trunk can be a risk factor for surgery, the anatomy of the aortic arch needs to be clearly depicted and described. If ultrasonography examination cannot exclude bicarotid trunk, enhanced magnetic resonance imaging or aortic arch angiography may be necessary.

Oualha M, Urien S, Spreux-Varoquaux O, Bordessoule A, D’Agostino I, Pouard P, Tréluyer JM.

Crit Care. 2014 Jan 24;18(1):R23.

Comment: According to this population analysis, the developmental effects of BW and age explained a part of the pharmacokinetic and pharmacodynamics between-subject variabilities of Ep administration in critically ill children. This approach ultimately leads to a valuable Ep dosing simulation which should help clinicians to determine an appropriate a priori dosing regimen.
