Prosthetic replacement options lack growth potential, and, in the case of a mechanical prosthesis, long-term anticoagulation is required. The Ross pulmonary autograft can provide excellent hemodynamics while maintaining growth potential and avoiding anticoagulation. However, the Ross procedure carries a risk of late root dilation without supportive techniques that would restrict growth potential in younger pediatric patients. As a result, aortic valve repair techniques for pediatric patients have gained increasing interest, and the German Pediatric Heart Center in Sankt Augustin, Germany, has long been a leader in advancing such techniques. In the present study, Hraska and colleagues review their program’s mid- to long-term outcomes of non-neonatal aortic valve repairs. The authors provide a nice review of the evolution of techniques for addressing aortic valve disorders in children.

 

Patients who underwent primary repair without previous intervention since 2003 at their institution were included. Patients were excluded if the aortic valve disorder was related to another cardiac or genetic abnormality (e.g., subaortic stenosis, ventricular septal defect, Marfan syndrome, common arterial trunk, transposition of the great arteries, Taussig-Bing anomaly, and/or congenitally corrected transposition). The primary endpoints were survival and freedom from reoperation and replacement.

 

The authors provide an elegant, detailed description of their operative approach, and helpful videos are included. Their approach typically included a series of steps to address repair and reconstruction. The first step involved leaflet remodeling to improve mobility including removing fibrous nodules and thinning the leaflets. The second step included suspending the leaflet apparatus to restore a functional geometry with adequate opening and competency. Occasionally this included augmenting or reducing the dimensions of the non-coronary sinus. Leaflet reconstruction was performed if the native leaflet tissue was insufficient or torn, and the authors used autologous pericardium exclusively. Their goal was to achieve or maintain a trileaflet valve where possible.

 

In all, 205 patients met inclusion criteria and 193 patients had adequate follow-up information.  The median patient age was 9.79 years (range, 0.1-28.7) and 50% were older than 10 years. Indications for repair included valve stenosis in 63%, regurgitation in 33%, and combined in 4%. Aortic valve morphology included unicuspid 17%, bicuspid 56%, and tricuspid 27% valves. Previous balloon dilation had been performed in 45% of patients.

 

The mean follow-up was 5.1 years. Survival at 1 and 10 years were 99% and 97.1%, respectively. Freedom from reoperation at 1, 5, and 7 years was 89%, 70%, and 57%, respectively (Figure 1). Reoperations were performed for stenosis in 25 patients, for insufficiency in 27 patients, for both in 6 patients, and for endocarditis in 3 patients. When looking at patient age and indication for reoperation, stenosis was the most common indication in younger patients (<10 years) whereas regurgitation was more common in older patients. Risk factors for reoperation as determined by multivariate analysis included age at surgery, previous balloon dilation performed before 6 months of age, absence of any developed commissure, neocommissure creation technique, a non-tricuspid post-repair valve arrangement, and cross-clamp time.

 

Freedom from valve replacement at 1, 5, and 7 years was 93%, 77%, and 68%, respectively (Figure 1). Of the 48 patients who required aortic valve replacement, the median age at repair was 8.0 years and median age at replacement 12.0 years. Risk factors for reoperation as determined by multivariate analysis also included previous balloon dilation performed before 6 months of age, absence of any developed commissure, a non-tricuspid post-repair valve arrangement, and cross-clamp time.

 

Impressively, 75% of the patients in this series were living with their native valve at last follow-up. Of these, 78% had a peak valve gradient <40 mm Hg and/or less than mild regurgitation. LV size and function were generally preserved.

 

The authors conclude that their strategy of aggressive early aortic valve repair can reduce or at least delay the need for valve replacement procedures in pediatric patients. Other factors for optimizing this ability as well as the durability of the repair include achieving a tricuspid repair and avoiding early balloon dilatation. By delaying aortic valve replacement, use of a supported Ross procedure or otherwise larger valve prosthesis were possible.

 

The main limitation of this study is its retrospective, single institution nature. With a median follow-up of 5 years, we cannot fully gauge how these patients fare as they enter adolescence and adulthood. It is not clear how many cases included multiple repair attempts within the same operation, especially since cross-clamp was a risk factor for reoperation. By excluding patients who had prior repairs, we also cannot determine how frequently repeat aortic valve repairs were possible. In this reviewer’s center’s experience, repeat repairs are possible using techniques similar to those described by the authors, though for a minority of patients.

 

Their results argue favorably for early surgical aortic valve intervention when indicated and avoidance of balloon dilation, which may disrupt leaflet integrity and diminish the potential for repair or its durability. Clearly, the refined and standardized approach at the authors’ center has yielded high-quality outcomes for this select group of patients. Multi-center studies and/or registries may help determine the practicality of this approach at other centers less accustomed to it.

Commentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  Neuro-developmental outcomes remain an important component of late outcomes after single ventricle palliation.  The potentially numerous perioperative and patient-specific risks related to single ventricle physiology have made accurately defining and measuring this parameter difficult. This group of authors from Germany aimed to compare neuro-developmental outcomes in preschool-aged Fontan patients. Specifically, they compared patients who underwent either a Norwood-type procedure (performed with deep hypothermia and selective cerebral perfusion) or who did not require neonatal surgery with cardiopulmonary bypass in the neonatal period.

 

In all, 131 Fontan patients ranging in age between 3.5 – 7 years met inclusion criteria.  Patients with genetic syndromes or other potential inability to accurately undergo neuro-developmental assessment were excluded.

 

Neuro-developmental assessment was completed in 95 (72.5%) of eligible patients. A German version of the Wechsler Preschool and Primary Scale of Intelligence-III was used to assess verbal, performance and full-scale intelligence quotient (IQ). Cognitive functions were assessed with a separate screening instrument. Patients were determined to have “impaired neuro-developmental outcome” if the result of either assessment was below average.

 

The Norwood group comprised 72.6% of the patient group with the Non-Norwood group comprising the remaining 27.4%. Norwood patients were younger at the time of initial palliative surgery (6 vs 11 days) and at the superior cavopulmonary anastomosis (3.3 vs 5.3 months). Following the initial palliative procedure, patients in the Norwood group had longer lengths of stay and a higher incidence of postoperative complications (43.5% vs 15.4%). One stroke occurred after each of the first two stages in the Norwood group.

 

Cumulative IQ and cognitive scores from both scales were similar between patient groups and were lower compared with population norms. The Norwood group exhibited a higher verbal scale score as well as a lower incidence of developmental delay compared to the non-Norwood group (17.4% vs 38.5%). Full-scale IQ was below average (<85 points) in 20% of Norwood and in 35% of non-Norwood cases. Global cognitive factor scale scores were below average (<16th percentile) in 28% of Norwood and in 38% of non-Norwood patients.

 

Risk factors for impaired neuro-developmental outcome in the entire cohort included head circumference z-score at birth and preoperative complications (e.g., multiorgan failure, need for intubation or inotropic support, etc.). In the Norwood group, these and postoperative stay were risk factors. In the entire cohort, patients who did not suffer a preoperative complication had higher overall scores than those who did (Figure 3).

 

The authors conclude that neuro-developmental outcomes in preschool-age Fontan patients resembled population norms. Furthermore, patients who did not require complex neonatal palliation (e.g., Norwood procedure) exhibited similar IQ and cognitive scores as those who did. They further surmised that smaller head circumference z-score at birth may indicate that abnormal brain developmental may begin prior to birth.

 

The single institutional nature of this study may be both a strength and a limitation. The uniformity of surgical management may help to minimize subtle variations in technique and perioperative management. However, it is difficult to know whether these results are generalizable to other institutions.

 

As the authors note, selection bias may exist with inclusion of only 72.5% of eligible patients and omission of non-survivors. In addition, without knowing the survival data for each stage, it is difficult to know whether their institutional outcomes are like other published data.

 

This study provides an interesting, contemporary insight into the natural history of neuro-developmental outcomes in patients with single ventricle physiology. Perhaps with current methods of staging and perioperative management, this outcome in this group of patients has improved over time. The authors note the emerging role of brain MRI, which is becoming a more frequent component of prospective studies evaluating neuro-development in these patients. Hopefully with larger patient cohorts, improved assessment capabilities, and longer duration of follow-up, we will continue to better understand this important facet of the complex single ventricle physiologic state.

jeremy-herrmannCommentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  This report from Elias and colleagues details a single institution’s evaluation of long-term quality of life for patients with cardiac disease who survived following a period of ECMO support. During an 18-year period, 396 patients were managed with ECMO with 170 (43%) known survivors. Patients and families of survivors were surveyed using the age-appropriate Pediatric Quality of Life Inventory (PedsQL). The authors received a 38% response rate to the phone survey and 19% response rate to the written survey with a median follow-up of 6 years.

 

The proportion of patients with single ventricle physiology was 36%, and 14% had no history of congenital cardiac disease. The most common indications for ECMO support were failure to separate from bypass or postoperative low cardiac output (combined 28%) and cardiopulmonary arrest (53%). The median age at ECMO was 78 days, the median duration of ECMO was 100.4 hours, and the median overall length of stay was 33 days.

 

In the verbal survey, 90% of patients or their proxy reported current health as “good” or “excellent”, and 86% indicated having mild or no physical limitation. The surveys also indicated that 23.3% reported below-average school performance, 46.8% reported a learning disability, and 4.6% indicated the presence of a behavior disorder. Additional therapy use was frequent with 43.1% utilizing speech therapy, 43.1% tutoring, and 16.9% a neurologist.

 

When compared with normative samples, self-reported and proxy-reported scores for children were lower for both biventricular and single ventricle groups (Figure 1). However, adolescent scores were not significantly different than the normative data (Figure 1). The authors report that in univariate analysis, no patient-related variables were associated with current health status or limitations.

 

The study includes a heterogeneous group of patients and comparisons were not made between those with or without congenital heart disease. For older patients with congenital heart disease (e.g., Fontan patients), quality of life may be affected by other health issues. We also do not know how many patients required further intervention (including advanced heart failure therapies) that may have affected future quality of life. It would also be interesting to know if there was any era effect as the care of single ventricle patients has continued to be refined. Potential response and institutional biases must also be taken into consideration.

 

These findings indicate that patients with cardiac disease who require ECMO support and survive may be able to achieve an acceptable quality of life at least in the mid-term. These patients may require significant therapies and educational support, however. Despite the limitations of this study, this information may be helpful for counseling patients and families about longer-term outcomes. Hopefully, ongoing prospective data registries will eventually include surveying for these important factors of quality of life.

Commentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  The management of advanced heart failure can include the use of vasoactive/inotropic infusions, ventricular assist devices (VAD), or both. Whether either the use of vasoactive infusions for VAD therapies improves functional status in pediatric patients prior to and after heart transplantation remains unknown. The authors used Organ Procurement and Transplant data for U.S. patients age 1-21 years with dilated cardiomyopathy who were supported with either vasoactive infusions or left ventricular assist device (LVAD therapy) who underwent heart transplantation. They compared patient functional status at the time of listing, at the time of transplant, and at one year following heart transplant.

 

A total of 701 patients were included: 430 (61%) supported with vasoactive infusions and 271 (39%) with an VAD at time of transplant. Patients were excluded if they were on mechanical ventilatory support, biventricular assist device support, combined LVAD and vasoactive infusion therapy, or extracorporeal membrane oxygenation at time of transplant. Multiorgan transplant recipients were also excluded.

 

Patients in the VAD group were slightly older with a median age at listing of 15 years in the vasoactive infusion group and 13 years in the vasoactive infusion group (p = 0.02). The percentage of patients ≥18 years of age in the vasoactive infusion group was 20% and in the VAD group 25%. There was a slightly higher percentage of patients listed status 1A in the VAD group (73%) compared to the vasoactive infusion group (67%). Discharge to home before transplant occurred for 46% of VAD patients compared with 26% patients receiving vasoactive infusions. VAD patients more likely to have renal dysfunction including need for dialysis at time of OHT and had a longer waiting list time (median 74 vs 28 days, p<0.001). Neurologic injury occurred in 3 LVAD patients and in one vasoactive infusion therapy patient (p=0.04). The types of VAD used were not specified though the authors report that in a sub-group analysis, there was no difference in FS between pulsatile and continuous-flow devices.

 

Functional status was assessed using the 10-point Karnofsky and Lansky scale. At the time of listing, the vasoactive infusion group had a slightly higher FS score than the VAD group. However, the FS improved in the VAD group but decreased in the vasoactive infusion group by the time of transplant. One-year following transplant, there was no difference in FS between groups. Overall survival at two years was also similar.

 

This study did not evaluate outpatient VAD programs, which may vary significantly between institutions and which may determine the discharge to home rate of patient. Rates of discharge to home were also were not evaluated by age group, and it is possible medical and device compliance may be better with adolescent and young adult patients. Other adverse outcomes of VAD therapy (e.g., thromboembolic events, antigen sensitization, etc.) were not available from that registry but could also factor into the risk profile of that management pathway.

 

The authors raise the question of whether a 1-2 point improvement in a FS scale is worth the possible additional renal and neurologic risks of LVAD therapy. At least in this editor’s anecdotal experience, school-age children who receive an implantable VAD prior to heart transplant may be able to resume some normalcy in life such as returning to school and re-engaging socially after discharge to home. However, this requires a tremendous amount of familial and outpatient VAD program support, which may not be as available at all institutions. Comparing quality of life between these groups may be helpful as would determining whether there is any cost difference between management strategies, especially in the outpatient setting.

Accordingly, options other than heart transplantation have historically been limited though there has been recent momentum in applying existing mechanical assistance technologies as well as developing novel technologies. An important consideration of failing single ventricle physiology is the distinction between those patients with ventricular dysfunction and those without. In the former, existing devices designed to augment ventricular function may be useful. However, these devices may have limited applicability for patients with preserved ventricular function but elevated systemic venous pressures.

This paper provides an excellent update of device therapies for patients with failing single ventricle physiology after any stage of palliation. The authors summarize key physiologic concepts and the experience to-date of mechanical circulatory support including pulsatile devices, continuous flow devices, and the total artificial heart. Limitations of existing device therapies for failing Fontan patients without ventricular function are discussed as related to their inability to mitigate elevated systemic venous pressures. Future directions for cavopulmonary assist devices (e.g., the Rodefeld pump) and other evolving technologies are also highlighted.

Commentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  The choice of technique for replacing a stenotic and/or regurgitant native aortic valve in a pediatric or young adult patient can be challenging. Bioprosthetic valves do not require long-term anticoagulation but have very limited durability in younger patients. Mechanical valves are more durable but require anticoagulation. Both bioprosthetic and mechanical valves also may be limited by lack of growth potential. Advantages of the Ross procedure (pulmonary autograft replacement of the aortic valve) include growth potential, avoidance of long-term anticoagulation, and favorable hemodynamics compared to prosthetic valves. However, the Ross procedure involves replacement of two valves and a more technically demanding operation. As a result, the Ross procedure tends to be performed in a minority of congenital surgical programs that perform it with regularity and maintain clinical competence with it.

The group from Leiden University is the latest to offer a report of their long-term institutional experience with the Ross procedure. Over a 22-year period, they performed 105 Ross procedures and 49 Ross-Konno procedures. The median patient ages were 14 years (range, 8-25 years) for the Ross procedure and 1 year (range, 0.25-9 years) for the Ross-Konno procedure. Overall survival was good (and similar to other reports) with 20-year survival rates of 90.7% for the Ross group and 70% for the Ross-Konno group (see Figure). Of the 26 patients (18%) who required reoperation for the autograft, 20 were related to autograft dilatation. As with many other centers, their technique for autograft implantation has evolved to counter long-term aortic root dilation. However, surgical techniques were not compared, though the authors observed no difference between age and time and autograft reoperation. Thirty-one patients (21%) required pulmonary homograft reoperation. For the Ross group, the 5- and 20-year incidence of pulmonary homograft reoperation was 5.8% and 30.5%, respectively. The need for pulmonary homograft replacement was predictably higher in the younger Ross-Konno group with a 15-year incidence of RV-PA conduit replacement of 50.5%. Only four patients in the Ross group developed endocarditis, all with the pulmonary homograft.

The major limitations of this study are its single institutional experience, retrospective nature, and potentially strong selection bias given their preference for the Ross procedure in this patient population. In addition, the lack of comparison with other similarly aged prosthetic aortic valve replacement patients makes it difficult to judge their long-term results. However, their results are consistent with other reports and indicate the Ross procedure is a viable option for younger patients who cannot take or wish to avoid long-term anticoagulation. Future prospective, contemporary comparisons (as with the German Ross Registry) may help to clarify these decisions and facilitate patient counseling.

jeremy-herrmannCommentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  While it is well understood that neonates who require ECMO support after a Norwood procedure have overall worse outcomes than those who did not, what is less well understood is how survivors after ECMO support fare in the intermediate term. The authors now offer insight into the functional performance of these patients. Using a previous prospectively collected patient cohort followed up to 7-8 years, they compared surviving patients who required ECMO support after the Norwood proceed with age-matched control patients who underwent a Norwood procedure without ECMO support. Validated inventories were used to assess functional status and quality of life. Interestingly, of all the items used to compare the ECMO and non-ECMO patients, the only difference was a lower perceived physical appearance score in the ECMO group. Consistent with other previous reports, both groups scored lower in functional status and quality than a healthy population. The authors conclude that patients who require ECMO support after a Norwood procedure and survive fare as well as those who do not require ECMO support in terms of functional status and quality of life. While the sample sizes of the present study are small, these findings may facilitate patient family counseling and provide a foundation for larger, longer-term studies evaluating these important outcomes in a very challenging patient population.

Commentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  This study is one of the latest to arise from the Pediatric Critical Care Consortium (PC4) database which is comprised of 14 centers that voluntarily submit clinical registry data for cardiac patients cared for in a cardiac ICU. The authors examined extubation failure (reintubation within 72 hours of first planned extubation) for 899 neonates who underwent cardiac surgical procedures. Extubation failure occurred in 11% of all cases. An extensive list of patient-related and perioperative variables was examined. Multivariable analysis revealed only the presence of an airway disorder to be associated with extubation failure even when failure occurred early (<24 hours) or later (24-72 hours). Patients who suffered early extubation failure were more likely to have longer ICU and hospital lengths of stay as well as higher in-hospital mortality. Additionally, there was a wide range of extubation failure rates across centers with two centers registering significantly higher than the average rate (5-22%; Figure 1). The authors reason that this method of cross-institutional reporting may facilitate adoption or transference of best-practices, especially when outliers are identified. The major limitations of this study are the typical limitations of any database-centered project, though the careful entry and validation of data in the PC4 database are certainly strengths of the program. It will be interesting to observe what additional practice pattern variations are identified through PC4 as the program continues to accrue more programs and enrolled patients.

jeremy-herrmannCommentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  Defining an optimal range of hematocrit values following cardiac surgery in pediatric patients remains a challenge, particularly for cyanotic patients undergoing placement of a systemic-to-pulmonary artery shunt. The Columbia group reports results of a retrospective analysis of their experience with these shunts from 2010-2015. Eighty neonates were included: 28 with Norwood with modified BT shunt; 39 with modified BT shunt; and 12 with a central shunt. The first postoperative hematocrit values were analyzed in comparison with shunt occlusion and 30-day mortality. Their standard postoperative anticoagulation regimen included aspirin but no systemic heparin. There were 12 instances of shunt occlusion, 5 within the first 24 hours (“early” shunt occlusions). Notably, none of these patients who developed early shunt occlusion died during the study period. There were 6 deaths in the series, 2 of which occurred after late shunt occlusion on postoperative days 12 and 18. On univariable analysis, the two factors associated with increased risk of early shunt occlusion were the first postoperative hematocrit level and later operative era (after July 1, 2014). Interestingly, the first postoperative hematocrit levels were higher in the later operative era, and for each additional 5 additional percentage points of hematocrit value, the risk of early shunt occlusion more than doubled (odds ratio [OR], 2.70; p=0.009). In terms of mortality, increased inotrope score and first postoperative PaO2 were associated with increased mortality in univariable analysis. Increased postoperative hematocrit was also associated with increased inotrope use and higher postoperative PRBC transfusion volumes were associated with increased risk of mortality, but no association between the first postoperative hematocrit and mortality was seen. The authors conclude that lower postoperative hematocrit values may be better for patients undergoing systemic-to-pulmonary artery shunt procedures, though the ideal range remains undetermined. They describe their practice of not transfusing PRBC for levels above 55%. With these results and those from other previous studies that do not strongly support or refute the routine use of systemic heparinization following shunt procedures, the authors continue to routinely use only aspirin in the postoperative period. The results of the present study are compelling regarding the avoidance of high hematocrit levels in the early postoperative period after shunt procedures. However, the question of how best to manage anticoagulation in these patients remains unanswered and may be beyond what a single institution, retrospective study can determine.

Commentary from Dr. Jeremy Herrmann (Indianapolis), section editor of Congenital Heart Surgery Journal Watch:  Fluid management of neonates following cardiac surgery can be challenging, and fluid overload can be difficult to avoid. In this study, the Cincinnati group conducted a prospective, randomized study comparing the use of intravenous furosemide or peritoneal dialysis (PD) for neonates with postoperative oliguria (urine output <1 ml/kg/hr over a 4-hour period in the first 24 hours). PD catheter placement was planned preoperatively for high-risk patients (age <3 mo undergoing cardiopulmonary bypass, age <4 mo undergoing tetralogy of Fallot or double outlet right ventricle repair, or age <6 mo undergoing heart transplantation) and selectively for other moderate-risk patients. The surgeon was blinded to initial group assignment. Patients were excluded if they had preexisting kidney disease, did not undergo cardiopulmonary bypass during surgery, did not have a PD catheter placed as planned, died during the study period, or required ECMO support or reoperation. Seventy-three patients were included, 32 patients in the furosemide group and 41 in the PD group. The authors observed no difference in the proportion of patients with a negative fluid balance on by the first postoperative day between groups, the but patients in furosemide group were more likely to develop a 10% fluid overload (odds ratio [OR] 3.0, 95% CI 1.3 to 6.9) and require prolonged intubation of more than 72 hours (OR 3.1, 95% CI 1.2 to 8.2). Patients in the PD group demonstrated better fluid balances on postoperative days 1-3, required less inotropic support, and exhibited fewer electrolyte abnormalities compared to the furosemide group. There were 2 adverse events related to PD catheters involving bloody drainage in one patient and a hydrocele in the other, neither of which required intervention. Pleuro-peritoneal communications were identified in 9 patients in the PD group based on drainage of dialysate from the chest tubes, and these patients did not undergo PD as assigned. No difference in mortality was observed.

 

The authors conclude that PD is safe and offers better clinical performance compared to furosemide therapy in oliguric, postoperative infants based on more favorable inotrope use, electrolyte levels, rate of prolonged intubation, and avoidance of fluid overload. However, they also conclude that their study was underpowered to evaluate other clinical outcomes parameters (e.g., length of ICU stay). Given the prospective, randomized nature of this study, these findings are worthy of consideration. Of further interest would be a cost comparison study of PD and furosemide treatment pathways and related clinical outcomes.
