Comment: This study further strengthens the deleterious effects of cardiopulmonary bypass in CHD and identifies some potential therapies of promoting white matter regeneration by stimulating oligodendrocyte progenitors after earlier surgery using deep hypothermia. Neurocognitive delays in ACHD patients is quite prevalent and studies like these may help change the early and long term neurologic outcomes.

Comment: Simple maneuver in the OR just after skin closure to recruit more alveoli and improve Pa O2/saturations with slight transient elevation of mean pulmonary arterial pressure. We can drop this article – the Study does not have a clear and strong message. It does not conclusively mention if this maneuver reduces the incidence of atelectasis, post op stay, pulmonary crisis etc.


3. Is there a role for mechanical valve prostheses in pulmonary valve replacement late after tetralogy of Fallot repair?

Comment: This review may be of less value in the era of percutaneous pulmonary valves however it gives an opportunity to think on this option as a last resort. The authors have very well complied pulmonary mechanical valve series published so far.

Comment: Both platelet count and platelet function were significantly reduced after CPB in children undertaken correctional heart surgery. Duration of CPB and hypothermia was associated with significant changes in platelet function.

Comment: The data demonstrate that early complete repair of PA/VSD/MAPCAs can be accomplished with low mortality and excellent postoperative hemodynamics. These early hemodynamic results are maintained at medium-term follow-up. We conclude that early complete repair is an appropriate choice for this highly select subgroup of patients.

Comment: Cardiac surgery performed on infants with low body weight is generally a complex procedure, but the results are acceptable. The risk was higher than that for patients with higher body weight in RACHS-1 category 3 and 4. Further investigation to improve the outcome of this high-risk group is needed.
