The impetus to perform percutaneous procedures in complex congenital heart disease patients to reduce the cumulative number of surgeries over a lifetime is steadily increasing. Dr. Lederman and group for the first time used real-time, interactive, anatomy guided, magnetic resonance imaging, facilitating a straight, passive MRI antenna-needle entry along a curvilinear trajectory from superior vena cava “donor” vessel towards the junction of MPA and RPA as “target” in fifteen naive swine. Iron oxide–tipped introducer sheaths, nitinol guide wires and 1% gadopentetate contrast was used for MRI visibility. The continuous, real time imaging obviated the need for a pre-positioned distal target balloon, snare, docking magnets or catheters previously attempted by different groups in animals to create shunts. In 10 animals, balloon-expandable off-the-shelf endografts secured a proximal end-to-end caval anastomosis and a distal end-to-side pulmonary anastomosis that preserved blood flow to both branch pulmonary arteries. For the next 5 animals, a novel, custom-built, self-expanding device was used. Intraprocedural real-time MRI, post-procedural MRI, x-ray angiography, computed tomography, and necropsy showed patent shunts with bidirectional pulmonary artery blood flow.
